Responsible for undertaking assessments for children with disabilities and complex needs.
Leading plans for children with disabilities and complex needs
You will be responsible for the implementation, monitoring, and reviewing of plans within multi-agency meetings relating to the assessed complex needs of children, young people, and their families.
Developing effective working partnerships is key with those within their families, carers, and significant othersAs a Social Worker you are required to:
Hold a Social Work England registration
Be eligible to work in the UK
Hold a full UK license
Post-qualified experience
