About the Role
You will manage a varied caseload, working directly with children with disabilities and their families to achieve positive outcomes and promote independence.
Your responsibilities will include:

Undertaking high-quality assessments and reviews.

Supporting children and families with complex needs.

Coordinating short breaks and joint-funded care packages.
*
Developing effective, child-centred support plans.
*
Working closely with health, education and other multi-agency professionals.

Advocating for children and ensuring their voices, wishes and needs remain central to decision-making.

Maintaining high-quality records and ensuring work meets statutory and professional standards.
