Description:
Case Work Coordinator required in Exeter within a charity that supports vulnerable and at-risk individuals who have a recent history of homelessness and are moving on from first stage accommodation.
£24,500 per annum
37.5 hours per week
Working 5 days a week and participating in an out of hours call rota, which may include weekend and evening working as required.
27 days annual leave, including an extra day off for your birthday, plus bank holidays.

About the role
Complete needs assessment/risk assessment for clients
Give clients advice, information and provide an enabling service so that they can be signposted to and access appropriate services.
Build positive and effective working relationships and agreements with relevant external agencies.
Progress and track referrals made into the service and outcomes for service users moving on from service.
Maintain client records that are accurate and fully updated
Pro-actively manage neighbourhood and community relations, responding appropriately to any concerns when they occur.
Manage the planned and positive move-on of clients through the service
The right candidate
Full driving license and access to a car for work purposes
Excellent verbal and written communication skills
Previous experience working as part of a team in a social or health care setting
Confident working with socially excluded people and those with complex needs and challenging behaviour
