Care Hub Coordinator

We are recruiting a Care Coordination Hub Coordinator on behalf of busy Local Coordination Centre supporting the delivery of nursing, hospice care services across the region. The work will be on a temporary assignment.
This is a non-clinical healthcare coordination role where your organisation, communication and problem-solving skills will directly support patients and families when reliable, compassionate care matters most.
You'll help coordinate care bookings, allocate nursing and healthcare staff, maintain accurate records and respond to changing priorities. Some days will run perfectly to plan. Other days will politely ignore the plan, and that's where you come in.
What You'll Be Doing
Working closely with the Senior Coordinator, clinical teams and external healthcare partners, you will:
Coordinate new and existing patient care arrangements from referral through to service delivery
Support planned day visits, night care and multi-visit care services
Identify urgent, emergency and rapid-discharge referrals and escalate them appropriately
Prepare documentation and keep patient, referral and service information accurate and up to date
Maintain daily and monthly staffing rotas
Allocate Healthcare Assistants, nursing staff and other care resources effectively
Book care visits and update cancellations, care packages and staffing changes
Respond quickly to last-minute rota changes and additional cover requirements
Liaise with patients, families and carers about care arrangements and service updates
Work with NHS teams, District Nurses, hospital discharge teams and palliative care professionals
Handle telephone enquiries professionally, sensitively and confidentially
Support the resolution of straightforward concerns and complaints
Ensure records are accurate, audit-ready and completed within required timescales
Raise staffing shortages, system problems and service pressures before they become bigger problems
Contribute to team meetings, service improvements and training activities
