About the Role

As a Coordinator, you will work closely with the Registered Manager and operational teams to ensure care and support services are delivered safely, efficiently and in line with individual needs.

You'll be responsible for coordinating rotas, supporting staff, maintaining service continuity, managing operational challenges and ensuring our service users receive the very best support.

This role combines people management, workforce planning, quality assurance and service coordination, making it ideal for someone who enjoys both operational responsibility and relationship building.

What You'll Be Doing

Care & Support Coordination

Coordinating day-to-day delivery of care and support services
Ensuring care packages are delivered safely and consistently
Responding to service changes, absences and operational challenges
Supporting the smooth onboarding of new service users
Promoting continuity of care through effective staff matching

Workforce Planning & Rotas

Creating and maintaining effective staff rotas
Coordinating planned and emergency cover
Monitoring staffing levels and escalating concerns
Matching staff skills, training and experience to service user needs
Working collaboratively to identify recruitment requirements

Supporting Quality & Compliance

Monitoring care records and documentation
Supporting care planning, reviews and risk assessments
Participating in service audits and quality checks
Contributing to CQC compliance and inspection readiness
Promoting best practice and continuous improvement

Safeguarding & Risk Management

Supporting safe practice across services
Identifying and escalating safeguarding concerns
Supporting incident management and learning
Promoting a positive, person-centred and rights-based culture

Staff Support

Acting as a key point of contact for care and support teams
Supporting induction and onboarding of new staff
Providing guidance, feedback and operational support
Encouraging high standards of professionalism and service delivery

About You

We're looking for someone who is organised, proactive and passionate about delivering excellent care.

You will have:

Level 3 Diploma in Health & Social Care (or equivalent)

Experience within health and social care, homecare, supported living or community services

Experience coordinating staff, rotas, schedules or care delivery

Strong understanding of safeguarding and person-centred practice

Excellent communication and organisational skills

Confidence using digital systems and electronic records

The ability to work flexibly across 5 days out of 7

A willingness to participate in the out-of-hours management on-call rota

We'd Love to See

Experience in a Care Coordinator, Senior Support Worker, Team Leader or similar role
Experience working with people with learning disabilities, autism, mental health needs or complex support requirements
Knowledge of CQC standards and regulated care services
Experience using care management systems such as Nourish or similar platforms
Previous on-call or senior operational responsibility
