Description:

The Care Coordinator is instrumental in overseeing the seamless provision of care services for our clients/patients by implementing efficient rota management alongside facilitating comprehensive care coordination. This role requires adept organizational skills, excellent communication abilities, and a compassionate approach to supporting individuals in need. The Care Coordinator collaborates closely with healthcare professionals, social workers, and other stakeholders to ensure optimal care delivery while advocating for the well-being and needs of the individuals under their care.

Key Responsibilities:

Assessment and Care Planning:

- Conduct thorough assessments of clients' needs, preferences, and goals.

- Collaborate with interdisciplinary teams to develop individualized care plans.

- Ensure care plans are regularly reviewed and updated to reflect changing needs and circumstances.

Coordination of Services:

- Liaise with healthcare providers, social services agencies, and community resources to coordinate care services effectively.

- Arrange appointments, referrals, and transportation for clients as needed.

- Monitor and track service utilization to ensure timely delivery of care.

Client Advocacy:

- Serve as an advocate for clients, ensuring their rights and preferences are respected.

- Address concerns and resolve conflicts to ensure the highest quality of care.

- Empower clients to actively participate in decision-making regarding their care.

Communication and Collaboration:

- Maintain regular communication with clients, family members, and caregivers to provide updates on care plans and progress.

- Collaborate effectively with internal and external stakeholders to promote continuity of care.

- Participate in team meetings, case conferences, and other forums to discuss client needs and coordinate care efforts.

Rota Management:

- Develop and implement rota schedules for care staff, ensuring adequate coverage based on client needs and staff availability.

- Monitor rota compliance and make adjustments as necessary to optimize staffing levels and meet client demands.

- Address staffing issues, such as absences or scheduling conflicts, in a timely manner to prevent disruptions in care delivery.

Documentation and Reporting:

- Maintain accurate and up-to-date documentation of client assessments, care plans, and service records.

- Generate reports and statistical data to track outcomes and evaluate the effectiveness of care interventions.

- Ensure compliance with regulatory requirements and organizational policies.

Education and Support:

- Provide education and support to clients and their families on accessing resources, managing health conditions, and navigating the healthcare system.

- Offer emotional support and counseling to clients during times of transition or crisis.

Qualifications and Skills:

- Qualification in social work, nursing, psychology, or related field.

- Proven experience in care coordination, case management, or a related healthcare role.

- Strong knowledge of healthcare systems, community resources, and social services.

- Excellent communication, interpersonal, and problem-solving skills.

- Ability to work collaboratively in a team environment and independently with minimal supervision.

- Empathy, compassion, and a client-centered approach to care delivery.

- Proficiency in computer applications and electronic health records systems.

- Certification in care coordination or case management (desirable).

Working Conditions:

- This position may require occasional travel to client homes, healthcare facilities, or community sites as far as Torquay/ Devon/ Cornwall.

- Flexible work hours may be necessary to accommodate client needs and participate in meetings/events outside regular business hours.

Please note- A clean full UK licence is essential for this role with the appropriate insurance cover for the role requirements.
