Tempting Recruitment are working alongside our client, a Local Authority, who are looking to recruit an experienced Care Assessor (Adult Social care) to join their team based in north-west London

JOB SUMMARY
The Care Assessor is responsible to evaluate and highlight varying needs for the purpose of different types of care support options, traditional care, reablement or supported living.
JOB DESCRIPTION
The duties and responsibilities of the role will include:
Assessing needs of vulnerable adults who have learning disabilities with complex health needs, challenging behaviour and mental health
Review and monitoring of Community Care Packages on a planned basis, as agreed.
Maintaining good liaison links with internal and external partner organisations to ensure better outcomes for individuals.
Respond in emergencies where packages of care are in danger of breaking down also when key worker is not available
THE SUCCESSFUL CANDIDATE
The successful candidate will have the following skills and competencies:
NVQ Level 3 or equivalent qualification or academic standard
Knowledge of social policy & relevant legislation, in relation to Adult social and health care improvements and of eligibility criteria for services.
Knowledge of social and health care needs of vulnerable adults with learning disabilities
Experience of working in Social Care Services or similar organisations such as the NHS or Voluntary Sector.
