AKTON Recruitment is working closely with the local authorities and we are looking for an experienced practitioner, to support our Front door team in Adult Social Care.

You will have experience of working within Adult Social Care and the ability to undertake strength based screening assessments in line with the Care Act 2014. The ability to work autonomously and ideally have experience of using Mosaic (however, training can be provided around this)

The role will involve undertaking screening assessments to determine the likely need of our residents, identifying the need for social work and/or OT intervention and escalating on to case management, where necessary. Providing information and advice to promote independence and connect our residents to community services. Self-allocation of work and providing updates on cases to our Deputy team manager.

You will join a busy front line team in Adult Social Care in Southwark. The team is the first point of contact for many of our residents. We aim to empower and connect our residents, using strength based approaches, promoting self-resilience and preventing the need for long term support.

The team is currently staffed by Customer Support Officers, 1 Occupational Therapy Assistant Practitioner, 3 Occupational Therapists, 3 Social Work Deputy Team Managers, and a Social Work Team Manager.

The post offers flexibility, with the option to work from home 2 days per week.

Responsibilities
1. Engage with, inform, and adapt to changing contexts that shape practice. Operate effectively within own organisational frameworks and contribute to the development of services and organisations. Operate effectively within multi-agency and inter-professional settings.

2. To be responsible for the day to day interpretation and implementation of statutory and departmental policy in the provision of casework and, if appropriate group work or other social work service, to individuals, families and other groups.

3. Undertake trusted assessment/provision functions to promote independence and, where appropriate, offer access to standard equipment, which may involve the issuing of an equipment prescription (for redemption via a retail outlet) and delivery/fitting.

4. Use judgement and authority to intervene with individuals, families and communities to promote independence, provide support and prevent harm, neglect and abuse
