Full time - 35 hours
At least 3 days per week office based

* We carry out reviews of care and support plans.
* Assessment needs under the care Act 2014
* Safeguarding concerns
* Mental capacity assessment
* We conduct home visits.
* Carers assessment
* Placement reviews
* The worker must have 2 to 3 years working in the adult social care setting.
