Social Personnel are recruiting for 2x Social Workers for a Transitions Team. This position is to work remotely for the majority but you are required to work one day in the office.

Essential Experience:
* Experience in a Transitions Team
* Experience working in Adults Disabilities
* Social Work England Registration
* Degree in Social Worker
