Adults Social Worker - Hospital Discharge - Queen Alexander Hospital
An exciting opportunity has arisen for a Qualified Social Worker to join our friendly and supportive hospital discharge team at Queen Alexander Hospital discharge team in Portsmouth. Position curretly available until June 24.
Responsibilities:
Working with the acute team.
The focus will be on supporting the turn around of patients at the front door and working with the Single Point of access here at the hospital.
This role requires you to be on site 4 days a week as you will be assessing patients for discharge planning.
1 day a week will be working remote covering another hospital.Our Social Workers are an asset and are encouraged to be creative and autonomous, while enabling you to work flexibly.
