Working Pattern: Monday to Friday

Hours: 37 hours per week

Hybrid Working: 2 days in the office, 3 days working from home

The Role

As a Qualified Adult Social Worker within the Hayes & Five Elms Locality Team, you will manage a varied caseload of adults with a range of needs, completing strengths-based assessments and delivering person-centred support plans. You will work closely with service users, families, health professionals, and partner agencies to safeguard vulnerable adults, promote independence, and ensure individuals receive the right support at the right time.

Your responsibilities will include:

Managing a diverse adult social care caseload.

*

Completing Care Act assessments, reviews, and support planning.

*

Undertaking Mental Capacity Assessments and Best Interest decision-making where appropriate.

*

Working collaboratively with multidisciplinary teams to coordinate effective care and support.

*

Identifying and managing safeguarding concerns in line with statutory guidance.

*

Maintaining accurate, timely case records and reports.

*

Promoting independence, choice, and positive outcomes for adults within the community.
